What to Expect During the Recovery Period After a Lumbar Epidural Steroid Injection

A lumbar epidural steroid injection (LESI) is a commonly performed procedure designed to reduce inflammation and relieve lower back and leg pain caused by conditions such as herniated discs, spinal stenosis, or sciatica. For many patients, this treatment provides meaningful longstanding relief with minimal downtime.

After your Injection

  • You may feel temporary numbness or weakness in your legs.
  • Mild soreness at the injection site is common.
  • Some patients notice immediate relief from the local anesthetic — this may wear off within several hours.

You will be monitored briefly following the procedure and before being discharged. Because of possible numbness and mild sedation, you should plan for someone to drive you home.

The First 24–48 Hours

  • It’s normal to experience mild discomfort at the injection site.
  • Some patients report a temporary increase in pain once the numbing medicine wears off. Ice packs can help reduce soreness.
  • Flushing or difficulty sleeping may occur and typically resolve in a few days.
  • Avoid strenuous activity, heavy lifting, or intense exercise.

Most patients can return to light daily activities the next day unless otherwise directed by their provider.

When Will I Feel Relief?

Steroid medication works by reducing inflammation around irritated nerves. Unlike the numbing medicine, steroids take time to work.

  • Pain relief may begin within 2–5 days
  • For some patients, improvement may take up to 1–2 weeks
  • Relief can last weeks to several months, depending on your condition

Every patient responds differently. The goal is often to reduce pain enough so you can participate more effectively in your daily activities.  After a lumbar epidural steroid injection, side effects can be mild and temporary, and typically resolve on their own. Serious complications are rare; patients should contact their provider if they experience any unusual or concerning symptoms, such as fever.

Activity and Physical Therapy

Many patients are encouraged to:

  • Resume light movement within 24 hours
  • Gradually increase activity as pain improves
  • Begin or continue physical therapy as recommended

The injection is often part of a comprehensive treatment plan, not a standalone cure. Long-term success comes from combining pain relief with strengthening and mobility work.

Follow-Up Care

It is important that you follow up with your provider after your procedure to:

  • Evaluate your pain relief
  • Determine if additional injections are appropriate; some patients may benefit from a series of injections spaced several weeks apart.
  • Adjust your overall treatment plan if needed
